Why do gums bleed? Common causes of bleeding gums

Bleeding gums are most often caused by plaque-induced gingivitis. Treated early it is usually reversible; left alone it can progress to periodontitis and bone loss.

Bleeding gums are most commonly caused by gingivitis, an inflammation of the gums triggered by bacterial plaque along the gumline. Healthy gums do not bleed easily when you brush or floss, so regular bleeding is a warning sign. Caught early, gingivitis is usually reversible with professional cleaning and better home care; if ignored, it can progress to periodontitis with bone loss.

A definitive diagnosis and treatment plan are made by a dentist after an examination.

What causes bleeding gums?

Plaque left at the gumline irritates the gums within days. They become red, puffy and bleed on touch. Over time plaque hardens into tartar, which holds even more bacteria. Gingivitis affects only the gums, but long-standing inflammation can lead to pockets and loss of the bone that supports the teeth. The 2018 AAP/EFP classification grades periodontitis by severity and rate of progression.

Other contributing factors

  • Smoking: it can mask bleeding, so disease may progress quietly.
  • Diabetes: Löe (1993) described periodontal disease as the "sixth complication" of diabetes.
  • Hormones: pregnancy and puberty can make gums bleed more readily.
  • Medication: blood thinners increase bleeding; never stop a prescribed drug without medical advice.
  • Brushing too hard: this can irritate gums and contribute to gum recession.
  • Rarely, general health problems: such as bleeding disorders or vitamin deficiencies.

When to see a dentist

  • bleeding that lasts more than two weeks;
  • swollen, dark red or tender gums, or pus;
  • persistent bad breath;
  • loose or drifting teeth, or teeth that look longer;
  • spontaneous bleeding or easy bruising elsewhere on the body.

Treating bleeding gums

At the examination, the dentist measures pocket depths around each tooth and may take X-rays. For gingivitis, a professional dental cleaning and home-care advice are usually enough, and bleeding often improves within a few weeks.

Periodontitis needs deeper root cleaning under local anaesthesia; some tenderness afterwards is common. Advanced cases may need surgery, followed by regular maintenance visits. See our gum disease treatment page. A 2005 Lancet review by Pihlstrom and colleagues highlighted plaque control as central to both prevention and treatment.

At home

Brush twice daily with a soft brush and fluoride toothpaste, and clean between your teeth every day. Do not avoid a bleeding area; gentle, regular cleaning is what helps it heal.

Frequently asked questions

Will bleeding gums go away on their own?

Mild gingivitis may improve with better brushing. However, tartar cannot be removed at home, so persistent bleeding needs a professional cleaning.

Is mouthwash enough?

Mouthwash can help but does not remove plaque or tartar mechanically. Some antiseptic rinses stain teeth with long-term use, so ask your dentist first.

Is gum bleeding normal in pregnancy?

They are common because of hormonal changes, but should not be ignored. Gum checks and cleaning can usually be done safely during pregnancy.

Are bleeding gums linked to general health?

Studies report associations between periodontitis, diabetes and cardiovascular disease. Whether one causes the other is still being researched.
