All-on-4 in Istanbul at our Bahçelievler clinic is a way of replacing all the teeth in one jaw with a fixed bridge supported by, typically, four dental implants. The two back implants are placed at an angle, which in many cases reduces the need for bone grafting. In suitable cases, a fixed temporary bridge can be fitted shortly after the implants are placed.

How All-on-4 works
The All-on-4 concept was described by Maló and colleagues for the lower jaw in 2003 and the upper jaw in 2005 [1, 2]. Two implants are placed upright at the front, and two are tilted at the back. Tilting keeps the implants away from the sinus in the upper jaw and the nerve in the lower jaw, while widening the support area and shortening the cantilever of the bridge [3].
Because of this, some patients with reduced bone can avoid sinus lifts or block grafts. It is not suitable for everyone, though; bone volume and quality must be checked with 3D imaging (CBCT).
Compared with a removable full denture, an All-on-4 bridge is screwed to the implants and does not come out, and in the upper jaw the palate is usually left uncovered. It does, however, require surgery and careful daily cleaning. If only a few teeth are missing, individual dental implants may be more appropriate.
Who is All-on-4 suitable for?
All-on-4 is usually considered for people who have no teeth in one or both jaws, people whose remaining teeth cannot be saved because of advanced gum disease or decay, and denture wearers struggling with a loose denture. If your teeth can be saved, that option is always discussed first – advanced periodontitis can sometimes be managed with gum disease treatment.
Extra caution is needed with uncontrolled diabetes, heavy smoking, previous head and neck radiotherapy, certain bone medications such as bisphosphonates, severe grinding or difficulty maintaining oral hygiene. A definitive diagnosis and treatment plan is made by the dentist after a clinical examination, CBCT and a review of your general health.
All-on-4 Istanbul: the treatment steps
- Consultation: medical history, medications and expectations.
- 3D imaging: CBCT to assess bone, sinuses and nerve position.
- Digital planning: implant positions and the teeth of the bridge are planned together; a surgical guide may be made.
- Surgery: under local anaesthesia, remaining teeth are removed, bone is reshaped if needed and four implants are placed.
- Temporary bridge: fitted soon after surgery when stability allows; otherwise healing comes first.
- Healing and reviews: suture check and early follow-ups with a soft diet.
- Final bridge: after integration, impressions, try-in and fitting of the definitive bridge.

What the research says
Our All-on-4 planning in Istanbul follows the published evidence. A study by Maló et al. with up to ten years of follow-up in the lower jaw reported high implant and prosthesis survival [4]. A systematic review by Patzelt and colleagues found favourable short- to medium-term outcomes but noted limited long-term data [5]. These figures come from selected patients; individual outcomes depend on bone, general health, smoking and maintenance. Smoking has been shown to worsen the prognosis of implant treatment [6], and the 2017 World Workshop consensus highlighted the importance of maintenance in preventing peri-implant disease [7].
Risks and aftercare
After All-on-4 in Istanbul, possible issues include swelling, bruising, infection, failure of an implant to integrate, fracture or screw loosening of the bridge, and peri-implantitis if cleaning is inadequate. Clean under the bridge daily with interdental brushes, super floss or a water flosser, avoid hard and sticky food in the early weeks and attend every review. Stopping or reducing smoking supports long-term implant health.

Frequently asked questions
Can I really get teeth on the same day?
In suitable cases a fixed temporary bridge is fitted shortly after the implants are placed. This depends on implant stability measured during surgery and on bone quality. If conditions are not right, healing comes first.
Are four implants enough for a whole jaw?
With adequate bone and careful planning, four implants can be positioned to support a full-arch bridge, and studies have reported favourable results. Some patients may need more implants depending on their bone.
Is the surgery painful?
Surgery is performed under local anaesthesia, with sedation considered where appropriate. Swelling and tenderness for a few days afterwards are normal. Taking the prescribed medication helps you stay comfortable.
Can All-on-4 be done if I have little bone?
Tilting the back implants can reduce the need for grafting in some patients. Whether this is possible can only be decided from a CBCT scan. Some cases still need additional surgery or a different plan.
How many visits will I need if I live abroad?
